Athletic Team Forms
Pre Participation Physical Exams  St. Agnes in partnership with Methodist Orthopaedic Specialists of Texas are teaming up to provide physicals for St. Agnes Students. If you need a physical to tryout out for sports you are invited to come out May 14th from 6-8pm. Additional information can be found in the form.
Permission Slip Mandatory for each athlete, allows athlete to practice/compete, turn in immediately upon making a team along with athletic fee.
Physical Print this form to take to your personal physician when you go for a complete sports physical.
There are 2 pages that complete the athletics physical form.
Every athlete must have the "Physical" form completed each year, and on file at St. Agnes before participating in SAA athletics. They can be turned into the Head athletic Trainer or to the head coach of their sport.
Athletic Policy  Mandatory for all athletes, players/parents should read this policy page carefully to understand the terms and requirements for all SAA athletes, due upon making a team.
